Before any crewmember enters a cargo tank or other enclosed space, there must be:
- AOnly verbal permission
- BA valid entry permit confirming the space is tested safe, ventilated, isolated, and attended, with rescue arrangements ready✓ Correct
- CNo testing if the tank looks clean
- DEntry first, testing afterward
Explanation
Enclosed-space entry requires a permit verifying the atmosphere has been tested (O2 ~20.9%, flammable ≤1% LEL, toxics below limits), the space ventilated and isolated from cargo/IG lines, an attendant posted, communications set, and rescue/resuscitation equipment standing by. Never enter on appearance alone.
